Современные подходы к лечению красного плоского лишая

Красный плоский лишай (КПЛ) - хроническое рецидивирующее воспалительное заболевание кожи. Несмотря на то, для лечения КПЛ назначается большое количество лекарственных препаратов, у части из них отсутствует четкая доказательная база, а часть лекарств может применяться не у всех пациентов из-за проблем с профилем безопасности. Цель этого исследования: проанализировать подходы к лечению больных КПЛ, чтобы помочь врачам выбрать оптимальный способ терапии таких пациентов. Обзор статей по лечению КПЛ проводился при помощи электронных баз данных Scopus, Web of Science, Medline, PubMed, elibrary.ru. Топические кортикостероиды и ингибиторы кальциневрина являются современными препаратами первой линии. В качестве других методов лечения используются системные и тоопические ретиноиды, метотерксат, циклоспорин, гидроксихлорохин, фототерапия и другие. К сожалению, отсутствуют крупные рандомизированные контролируемые исследования для подтверждения эффективности, безопасности и переносимости некоторых методов лечения КПЛ, и многие рекомендации основаны на исследованиях с небольшими размерами выборки, отсутствием стандартизированных показателей результатов, контролируемой продолжительности лечения. Следует отметить, что во всех случаях лечение больных КПЛ должно быть комплексным, индивидуальным. В то же время необходимо уделять внимание сопутствующим заболеваниям пациента, которые нередко осложняют течение кожного заболевания. Прогноз обычно благоприятный. Назначение лечения пациентам с КПЛ должно основываться на персонифицированном подходе к каждому пациенту в зависимости от степени тяжести, формы, локализации и клинической картины.
